The player controls a roster of four drugged indentured servant girls as they do their assigned chores across an empty mansion, with false promises of freedom. However, each and everyone is gradually picked off by an unseen monster, leaving only Margaret left.
By reading notes, the player can make out the game's backstory. The servant girls are test subjects for Doctor Edward Sullivan, the owner of the estate and a World War 1 veteran, in his ongoing experiment to summon some eldritch God he's been getting visions of through a PTSD-induced fever-like "Heat". This Heat spreads across the world like a virus and brings forth a cult of fanatics dedicated to God. Sullivan surgically resurrects the dead in his hopeless attempts at providing the God an avatar to possess, littering the mansion's underground laboratory with zombie-like monsters.
Margaret fights her way through Sullivan's ungodly creations with a rifle and escapes the mansion's grounds.
